New Passport
* Kindly refer to the
Passports General Guidelines and
Area of Jurisdiction.
The original copies of the following documents
should be provided:
- The Lebanese identity papers, plus one photostat
copy: either an I.D. Card or an Extract of Individual Civil Registry
Record (Ikhraj Kayd Fardy).
- Current Lebanese passport or last Lebanese passport
used.
- A
Passport
Application Form (click to download), in duplicate, duly
and completely filled in English and/or in Arabic.
- An imprint of the right hand thumb only of the
applicant on 2 ‘White Card’ (Batakat Ta’ryf) forms supplied by
the Embassy, (notarized, if sent by mail – see /download
Note*
hereunder). These cards are for official use and should not be filled
by the applicant.
- 3 Recent 4 x 4 cm. passport photos, (notarized, if
sent by mail).
- A Money Order or a Bank Cheque payable to the
“Embassy of Lebanon” in the amount of:
= A$ 62.50 for one
year OR =
A$ 312.50 for five years
- A Certified self-addressed, stamped (or Priority
Prepaid) return envelope.
Note*
: The 2 ‘White card’ forms (Batakat Ta’ryf) should have the
imprint of your right thumb undertaken in the presence of a Notary
Public and attested by him as follows:
“Before me, a Notary Public / Justice of the Peace, in
and for County of _____________, personally appeared
Mr./Mrs._________________, holder of Lebanese Passport (or Australian
Driving Licence, No. ..…), issued in ……….… (place), on …….… (date)
signed and affixed his right hand thumb on the annexed White Card
Form.

Renewal Passport
* Kindly refer to the
Passports General Guidelines and
Area of Jurisdiction.
The original copies of the following documents should
be provided:
- The Lebanese identity papers, plus one photostat
copy: either an I.D. Card or an Extract of Individual Civil Registry
Record (Ikhraj Kayd Fardy).
- Only recent
Lebanese passports with burgundy color cover can be renewed; holders
of older versions must apply for a new passport.
- A
Passport Application Form, in duplicate, (click to
download) duly and completely filled in English and/or in Arabic.
- An imprint of the right hand thumb finger of the
applicant on 2 ‘White Card’ forms (Batakat Ta’ryf) supplied by
the Embassy, (notarized, if sent by mail – see /download
Note*).
- 3 recent 4cm x 4 cm. photos, (notarized, if sent by
mail).
- A Money Order or a Bank Cheque payable to the
“Embassy of Lebanon” in the amount of:

Lost/Stolen Passport
* Kindly refer to the
Passports General Guidelines and
Area of Jurisdiction.
* In addition to
the documents mentioned under “New Passport”, the following documents
are required:
- A
complaint should be lodged with the local police and a Police Report
obtained.
- A
copy of a Notice published for 3 days in a local newspaper.
* Either a new passport valid for one year only or a
Laisser-Passer may be issued as each case will be considered on its
own merit.
* Such incidents point out to the necessity for
registration with the Lebanese Mission at country of residence or
nearest, at all times.

Laisser-Passer
* Kindly refer to the

* In most cases, prior
approval from the Relevant Authorities in
Beirut
to issue / renew Travel Document may be required (Allow 2-3 months for
a reply).

- All identity papers from Lebanese Authorities, plus a
photostat copy.

Laisser-Passer
or last Laisser-Passer
used, plus a photostat copy of front pages up to page 8.
- A

Adding a child / children ‘s’ name(s) on
passport
* Kindly refer
to the Passports
General Guidelines and
Area of Jurisdiction.
1. Specific requirements:
* The father of
the child should be Lebanese.
* The child’s
birth should have been registered either with the Embassy or with the
Civil Acts Register in Lebanon. (If not, proceed with the registration
process – see “Birth
Registration”).
* Minors who are
15 years old or younger may obtain an individual passport or Laisser-Passer
(see ‘Passport General Guidelines’ above) or have their names added to
one of their parents’ passport. They will not be able to use it to
travel alone without the parent accompanying them.
2. The
original copies of the following documents should be provided:
- Once the
registration is established, a letter from both parents authorizing
the Embassy to add their dependent(s) to either one of their Lebanese
passports (notarized, if sent by mail).
- 3 recent 4cm
x 4 cm. photos, (notarized, if sent by mail) of parent with
child/children.
- A Money Order
or a Bank Cheque payable to the “Embassy of Lebanon” in the amount of
A$23.50 per child.
- A Certified
self-addressed, stamped (or Priority Prepaid) return envelope.

Adding wife’s name to Passport

1. Specific requirements:
* The husband &
wife should be Lebanese.
* The Marriage should
have been registered with the Embassy or with the Civil Acts Register
in Lebanon. (If not, proceed with the registration process – see "Marriage
Registration”).
2. The
original copies of the following documents should be provided:
- Valid
Lebanese passport of the husband.
- 3 recent 4 x
4 cm. passport photos (notarized, if sent by mail).
- A Money Order
or a Bank Cheque payable to the “Embassy of Lebanon” in the amount of
A$ 23.50.
- Certified
self-addressed, stamped (or Priority Prepaid) return envelope.

Issue / Renewal of Lebanese
Travel Document for Palestinian Refugees

1. For holders of Travel Documents
issued before 1st January 1990,
their applications will be
forwarded to the Relevant Authorities in
Beirut for their
prior approval (Allow a period
of two to three months for the reply).
The original copies of the following documents should be

Documents:

have their names added to one of their parents’ passport. They will
